"Dear Dr. Tutera,
I am going to be 50 next month, and I am in the menopause or
pre-menopause stage of life. I started getting hot flashes probably at the end of last year along with irregular periods accompanied with the worst cramps I have ever had. The hot flashes seemed to go away and I haven't had a period for a few months now. About two weeks ago the hot flashes returned with a vengeance. I get so many of them I actually get headaches and feel sick to my stomach. I am taking estroven (over the counter) and using a natural progesterone cream. Unfortunately it doesn't seem to be working as well as it did in the beginning. Should I be taking a prescription medication instead? Aren't there all kinds of risks as a result of hormone therapy? I'm really trying to tough this out but it's starting to be quite a challenge. Any helpful advice would be greatly appreciated at this point.

The risks of pharmaceutical hormones are well documented, and may include a potential rise in clotting factors in the blood which can lead to stroke and pulmonary embolus (clot in the lungs) which can lead to death. Other risks include possible increased risk of breast cancer and osteoporosis. The progesterone cream you mention is also a concern. Progesterone has been given an undeserved, almost mythical status in the world of hormone replacement. Progesterone exists in the body as a transitional hormone. The body uses it to make other hormones. Unfortunately, Progesterone cream is not nearly as effective as it's publicized to be at protecting against heart attack, stroke Alzheimer's or osteoporosis.
What you are using does not protect your heart, brain, or bones. You need to find a physician who will give you Bio-identical Hormones which carry a much lower risk.
